Don't know whose toes you think you may be stepping on here Geoff but
they're certainly not for a start, mine.

Medium format digital seems to break some of the rules.
You have to really use very low ISO's.
I have no clue why. It could be the heardbreak of PPS.
PIXEL PITCH SIZE

Other things inside a camera than pixel sites.
Here's a description of the new D3s from robgalbraith.

"The sensor's basic specifications are the same as the earlier model,
including a capture area of 36mm x 23.9mm, a pixel pitch of 8.45?m,
12-channel readout and FX Format image dimensions of 4256 x 2832 pixels. An
improved microlens array on the sensor, plus revised internal circuitry
meant to counter image noise, contribute to the D3S' ISO range boost, says
Nikon USA's Silverman. "

again:
"An improved microlens array on the sensor, plus revised internal circuitry"

To me they are saying these pixel grabber thingies need backup.
Just like a pitcher needs a catcher and a third basemen.
