Cosina (Voigtlander) themselves manufacture these adapters, you should be 
able to get them from the same place that the lens came from. If you need 
specific names then try https://secure.ffordes.com/index.htm
You'll find they list the adapters at 34.99 GBP, if you're elsewhere than 
the UK I'm sure someone else will reccomend where to go.
