yes Ted...scary and entirely true !

people afraid to make a decision, including top doctors and  
administartors, who don't want to be left "holding the bag"...

so they "pass it up the chain of command..."

then they are surprised when they are stripped of all power to make  
any decisions...

and the chain of command results in some top administrator/and lawyer  
who says (and entirely predictably)

well... "just to be safe"  we better get...

blah, blah, blah...
