Geoff,

I did it this way mainly because, for me, it is more troublesome to blur 
in PS and I have never really liked the blur PS gives. A real optical 
OOF looks better, to me, than the PS created ones.  Doing it this way 
took maybe 5 minutes tops.  Open both files, select the shallow DOF  
one, paste on top of the high DOF one and then a simple layer mask, a 
little painting with black ink using the WACOM tablet, save PSD, 
flatten, resize save for web and done.

I did shoot to make an HDR but never put it together, saw no reason as 
this looks fine to me.

For those who are really good at PS the way you described probably would 
be easier and quicker.
