Hugh,
I think the vertical patch is something you'll have to work out for yourself 
by holding the camera up and see if it's the way you hold it in vertical. 
You might find tilting camera back and forth slightly until you find the 
right angle for your eye and patch.

Much like the "flair problem" some folks have with M camera's. When this 
happened to me I just shifted my eye slightly and the flair went away and I 
kept right on shooting.

This doesn't mean there isn't a flair thing as it's quite real, it's just a 
fast and easy way of getting it out of the way in the middle of a shoot.

I look for a highlight on the subject, maybe a tiny one on the edge of their 
glasses or in the eye if they do not have glasses. Even one on the skin of 
the cheek or nose will work.

But a highlight on any part of a subject is what I look for in low light 
situations and generally the range finder separates quite well.  Obviously 
you hope the light will be right on your point of focus for the picture 
composition.
