Brian,

I just opened the image in cs2 and the first thing i noticed is that  
the profile for the image is hfa2400photoragmk.  So it looks like you  
have the profile of the paper tagged to the image which is something  
I would never do.  I think you are double profiling.  try converting  
the image profile back to rgb or srgb and reprinting.
