I already read from Tina that it means 'i love you', but if I hadn't, I would have looked it up myself because the image is so intriguing. I think it's because his eyes are closed and because of his expression. I interpret this as he must be really suffering. In combination with the sign, it tells me he must be a very positive minded and courageous boy.
