I second the recommendation of A&I.  Excellent service.  I have found
them to be good at following special instructions and very responsive
to email questions.  Turn-around time met my expectations the 5 or 6
times I've sent them batches of film.

You can obtain A&I mailers direct from A&I or from a number of online
retailers at a slight discount.
