Yes, that particular shot and most other pictures I have been doing w/ flashes have TWO lights, 45 degrees to the side, as you surmise. In the paragraph above, I was referring to another time when I did it with only one light, 45 degrees. I didn't like that as much as two... but as I said, looks like you have plenty of food for thought for me.
