It's happened to me only once: in east Africa in the Ngorogoro Crater where I lost a roll of Kodachrome with the only "shots" I didn't take of a veritable flood of pink flamingos. Always ensure by running a finger across the sprockets that the film perforations fit over the sprockets and ALWAYS watch the rewind crank when you wind on the first two frames.
