You are probably suffering from the same thing that fools the non flash
metering, that is the tone of the object.  If the average reflectance of
the object is not 18%, the exposure will be off.  For example if you are
shooting a white marble statue, it will turn the flash off too soon and
make it appear grey.

The second problem with flash is the light falls off drastically from
foreground to background.  If the scene you are iluminating is not all in
one plane , the foreground objects will be over exposed and the background
underexposed.
