I'll let Tom A. answer part of this as I dont know the answer to No.1.

But the little leaver that sticks out is in the base plate and it folds out
while you are shooting and that is what you close the fingers of your left
hand around and squeeze to the left advancing film and re-cocking the
shutter after each exposure.

It is a magical piece of workmanship and a joy to use by any Leica user
that the Rapidwinder will fit their gear.  However the left eyed
photographer shouldn't be without it! This is where it really shines for
those whose prime eye for looking through the M camera is the left eye.

It means the camera never has to leave your eye to change from frame one to
thirty six as you can continuously fire away at whatever things you want. 
I don't mean 36 frames of the same thing obviously. 

Some photographers when they get into a shoot situation constantly view the
scene through the viewfinder and with the Rapidwinder you can follow  the
subject without ever taking the camera away from your eye. 

The above remark mainly applies to left eyed shooters, as the right eyed
guy can do it with out poking his eye out using the normal advancing lever
of the camera.
