Hi Eddy,
Initial reaction to your question? "You've not looked inside the camera bags
of many pros?"  :-)

My case on assignment? The subject dictated what cameras and lenses?
On my neck resting on my chest?
3 M bodies; neck straps set one above the other without  banging on each
other. 
Top camera:  21mm
Middle :  35mm
Bottom: f 1.0 50mm Noctilux.
LEFT SHOULDER: 2 R motor driven bodies.
top camera: 90mm.
bottom: varied?  a 100mm -- 180mm or the 250mm?
RIGHT SHOULDER
Motor driven camera: 280 2.8 
If a bit more reach? 1:4 & 2X extenders on 280mm? It became 800mm f:8:0.?
This wasn't equipment carried on every assignment. Usually doing a
documentary?
