In the medical corps, advancement was often different than in other ranks.  
Could be that the rank would be accurate for what we term a Practical Nurse, 
while a Registered Nurse normally is ranked a lieutenant.  
I'm guessing about the first part, but know the second part to be accurate. 

(Mom was a nurse in military civil service, but most of her co- workers were 
officers.)
