It's difficult for me to envision where this error arises, since the 
camera's autofocus optoelectronics examine the actual image, not a surrogate 
like a split-image RF does.

Unless you are using live view, the AF system examines the image at a 
surrogate image plane.  Calibration is as important as it is with the 
rangefinder.
