The best guide was Erwin Puts guide to versions and differences. 

Sadly, now missing from his Website. 

V3 was Canadian. I have one. I just checked EBay for a picture of V4 (easy 
to tell apart as V4 has tabs on the iris ring, V3 does not). That lens 
clearly states Canada as well. 
It may be true that some V4 were made in Germany but we can be sure that 
some were made in Canada. 

When Leica started to pull out (Got thrown out due to business decisions of 
Hughes/ Raytheon) of ELC, there was a transition period before the designs 
were changed and the ASPH and APO lenses versions came out. 
Some stuff was made in Germany when traditionally they were made in Canada.
