the skull "window" is made first by cutting the bone with a power saw,  
then the circular piece is removed, saved, to be replaced  
later...sometimes its removal is tough, and it needs a little  
coaxing,  as here...

if you hang these images in the waiting room, or in the metro,  it  
will help keep the cost of health care down...
