I have been using Extensis Portfolio for some considerable time now, and 
I personally think it is hard to beat. It handles around 10,000 images 
for me in all kinds of locations and it does it faultlessly and very 
quickly - but, like all these programs, it is as good as the information 
(keywording etc.) that you put in.
