Steve,
In my case, I required the installation of mechanical aortic and mitral
valves, and the work was done by a very skilled and experienced surgeon. 
It's hard for me to believe that the extensive work he did could be done
without opening the chest.  At least, he had a first hand look at what he
was working with.  I've had no problems and no regrets, except for the
lifetime use of Coumadin that comes with the territory in cases like mine. 
As an engineer accustomed to evaluating data of all sorts, I use a
spreadsheet to track and manage my Coumadin doseage.  My cardiologist
always expects a copy of my tracking curves when I see him once a year.
