I don't know that there is any set formula.  I usually adjust the
settings so that the blur softens the grainy areas and the sharpening
bring things back into 'focus'.  I also use Gimp as my image editor on
a Linux system; it doesn't use the same settings as Photoshop for
either control.  There are grain reducing plugins, I think, that might
do this better and more consistently.  Again, I do not use Photoshop,
so if that is your poison, I can't help point you toward something
that will work for you.

Did you adjust the image since yesterday?  If you did, I think you did
a good job of removing the grain.  Did you rescan or use Photoshop to
tone it down?  Looks better this morning, but it could be the
difference in monitor between my system at work and the one at home on
which I'm viewing it now.
