The distinction is kind of fuzzy. One could argue for example that GWB was appointed president by the actions of the Supreme Court. In England, the Prime Minister is 'invited' by the monarch to form a government on the basis of election results but is not directly elected... the Monarch remains constitutional head of State. In many countries the prime minister changes several times between elections because coalitions shift. This does not mean those countries are not democracies.
