Agreed... I really do think it was the lens and maybe a differing part, like
a shim or a different helix used.  Proving it would be relatively
impossible...

What I could tell is that the lens did focus correctly when it got back to
me.

That would worry me if it was the body...since they EVERY lense you have
should be sent in with it and ALL of them "mated"...

I will consider my self fortunate then that mine worked perfectly right off
the bat!  For what these things cost, they ALL should work right off the bat
;-/

FYI, I bought both my M6 body and 75/1.4 used.  Both were barely used, if
even for more than one or two rolls!
