Randy,

The 3rd cam is a stepped thingie next to the "inboard" side of the 2nd cam.  The 2nd cam is the one that is at the bottom of the lens mount when the lens is on the camera.

One easy way to tell if a lens was originally built with the 3rd cam is the focal length engraving in orange next to the DOF scales - if it has this marking, it was originally built with the 3rd cam.  If it doesn't have this marking it may or may not have the 3rd cam and you have to look at the cams to be sure.
