I use the Domke "Postal" shoulder pad together with the Gelderburn (a Brady
Fishing Bag),
<http://www.smithscountrypursuits.co.uk/products/brady.htm>
and the Hadley Removable Insert (Billingham)
<http://www.billingham.co.uk/web/html/products/photo/photo_hadley.htm>

The bag strap has exactly the width of the Postal shoulder pad. (much larger
than the Billingham strap)

A great combination for:

M6 + 90/2 (mounted) , M6+ 24/2,8+ viewfinder and 35/1,4 Asph.coupled with
50/1,4 + SF20

or

R8 + motor drive + 80/1,4 (mounted) + 80-200 (or 70-180 or 180/2,8) + 19/2,8
coupled with 35/2 + SF20
