When shooting transparencies, I meter for the 18% gray and then meter for 
the brightest part of the scene I want to see details in. If the 
brightest part of the scene that I want clear details in is more than 2 
EV higher than the 18% gray requirement, I reduce exposure down to that 
point.

I guess in Zone system type parlance, I'm placing my brightest details in 
Zone VII or VIII, given Zone V is 18% gray
