No, you're not the only one with strap lug problems.

I just got back my backup M4P with replaced strap lugs.  After losing 
one:  During a break from taking pictures of a business meeting, I had 
the M4P - carrying a 21/2.8 Elmarit! - slung over my shoulder.  On the 
way out of the hotel bathroom, I felt the strap give way.  Before I 
could grab anything, the outfit bounced off the tile floor.

After getting my breath back, I picked up the camera, lens still 
attached, and looked it over.  Damage was a good knock on the corner of 
the rfr housing, but the lens didn't show any thing. 

What had happened was, one of the strap lugs completely, and suddenly, 
pulled out of the body and the camera and lens simply slithered off my 
shoulder.  Not a good feeling. After the repair I asked Leitz NY if 
they had seen this before. No reply.  I'm not sure how the new ones are 
attached, but you can bet I hold onto it ALL the time!
