Ric, the older I get, the slower my memory seems to be.  They are 
probably just re-inventing the the "Zero Defects" program that was very 
popular with corporate Quality Control and Safety Department managers 
about forty years ago.  I think it was originated by Martin-Marietta.  
I've still got lapel pins somewhere in a dresser drawer.
