If the only penalty for a copyright violation is a request to remove an image, why would they care? They'd just yank the offending image. YouTube is a perfect example: I can readily see lots and lots of copyrighted material, including terrific movies, TV shows, etc. that obviously is filched (a favorite: the tag line in the description often reads "no violation of copyright is intended.") Eventually the copyright owner cries foul, and the video is yanked. Like cockroaches, the videos usually reappear soon after, under a different account name and perhaps a scrambled title. Many years ago (in the pre-digital media era) an attorney told me that he had a client (who he called "a character") who ran a rock concert t-shirt business. ALL of the client's products were made without benefit of appropriate licensing. The client would start selling shirts at the very beginning of concert season (early May), and continue until he was slapped with a cease-and-desist order, typically in July. They would postpone the hearings until the Fall, by which time the company was out of business with no assets. Without a deep pocket, the lawsuits were pointless. The client would then begin the cycle again the following Spring. The attorney told me that this charade only ended because the Feds became interested in the client's pharmaceutical entrepreneurship, and would probably have used the t-shirt business as a way to get the client behind bars, a la Capone and his income tax evasion conviction. Modern technology just makes IP theft that much easier to execute. Short of draconian punishments, theft will only continue to flourish.
