I use a pad with built in wrist pad.
Otherwise my hand falls off and all I have left is a stump.
But it gets gunky and I have to clean it off and blow off grit with an air
blower.

A thing like a Kensington
SlimBlade Trackball is an alternate solution but I cant "draw" with them as
I need to just  using a brush tool in Photoshop.

It just sits there. So its not exposed to a desk surface which is going to
have some grit on it.
