This is the on camera flash in a cavern with 40m open space above, 50m to 
"subject" and 50m of open space each side ;-)

Not always, but requires several flash units - a friend of mine does this 
with spectacular results in unlit caves but it requires another type of 
gear, and a fairly good physical condition ...
