More of Faviana's right arm makes the image more dynamic, and emphasizes whatever the hell it is she is doing to the patient :). I'd probably fit in there - Novocaine doesn't work on me so I just man up and sit through it. Nice colors, I wouldn't change anything except maybe Photoshop some blood running from the mouth.
