Same thing for me.  The herniated discs in my lumbar region put an end
to my heavy camera gear hauling many years ago.  

I am currently using the Lowepro Digital Resolution Beltpack AW [all the
logos trimmed off] with good results, but I'm always on the lookout for
the "next best" waist pack, much to my wife's chagrin ;-).  The DR
Beltpack has enough space in the main compartment for an M6/Noct combo,
two extra lenses w/viewfinders, nine rolls of film and an SF20 flash
[but it's a real *tight* fit].  The tiny outside compartment packs extra
batteries, self-timer, remote release, backup light meter, ND filters
and cleaning supplies.  The bottom opening [which contains the built-in
rain cover] has enough extra space for my mini ballhead and table top
tripod.

As good as the DR Beltpack is, it does have some shortcomings that I
would like addressed; namely, no water bottle sleeves, needs better
construction and just a little bit more space.  Recently I have been
looking at the OutPack waist pack as a possible replacement.

just to put a feminine perspective on this bag thread...I gave up the
shoulder/backpack things long ago. I use a waist pack that opens away
from me so I have a little surface support when changing film, lenses
and completely visible access to bodies and lenses passport and money
(rare!) The one I use will take my R6 and 3 (35/80/70-110)lenses or R6,
2 lenses and range finder(cont***G1-someday to be an M6).
Hmmmm...haven't tried that 2X extender yet. The minilux is in my
pocket.There are cool places in the lid for film, batteries and pockets
on both sides as well. And I can always see my stuff. I think mine is a
Lowepro. No sore back or shoulders but it doesn't help the knees... I
also travel with a padded lunch bag that I can put an ice pack in for
extra film. The bags that open away from your body are MUCH better.
