Try Stadium Opticians in Ann Arbor, Michigan.  They may be able to make you
one.   They ground me a Series VI enhancing filter (from the proprietary
Howard Ross glass) to fit a Hoya Series VI retaining ring mount, which is
itself something of a feat.  I'm sure that a diopter could be done too.
