I can think of four possible causes of this problem:

1. Improperly terminated SCSI bus. There are lots of ways to get
termination wrong, and only one way to get it right.

2. Duplicated SCSI address. You probably already thought of this.

3. Mismatch between the LS-2000 configuration and the SCSI card's
expectations. 

4. Incorrect Windows driver for this device and/or SCSI card.
