TIFF for sure!
Don't use jpeg except for the web.
Grayscale scans for B&W
Scan at the highest optical resolution of your scanner.
You might try scanning at 12 bit into Photoshop if your gear supports 
that.
That means no correction via your scanning software, you correct in 
Photoshop starting with Levels command.
