The German company doing these conversions is

Fotohaus Schaja/Photo Classics
Maximilianstrasse 32
D-80539 Munich
Germany
phone 0 89 - 291 624 83, fax 0 89 - 292 506.

The price for the conversion in 1996 was 1.890 DEM - or roughly 950 Euros. The price for the Hologon was approximately 5.000 DEM - and the lens can be converted back to Contax bayonet. All of the above according to Color Foto September 1996.
