In addition to a small bag, (and only when not on a "tie wearing" shoot) I use USArmy "BDU" (Battle Dress Uniform) "Blouses" (the standard US combat uniform shirt.) They come in Black, Khaki, Olive and Navy (and even the more traditional camouflages if your shooting wildlife in any region.) You can get them in 100% ripstop cotton weave or a heavier 60%Cotton 40%Polyester twill. I have several blouses with sleeves and several vests that I've removed the sleeves (taking your time and cutting correctly you can have a vest that looks tailor made and is still completely functional.) I like the longer "elbow reinforced" sleeves left on for cooler times of the year. The old M56 jacket also makes a great gear holder for the winter months with the same pocket layout, but a bit more padding and lots more warmth. They have four expandable button flapped pockets (no padding) all on the front that will hold an incredible amount of gear, no annoying zippers and (here's the best part) cost only about $19 each if you look in the right places. (http://www.cheaperthandirt.com for example) Again, unless you shoot extremely lightly, and don't mind unprotected pockets, you will also need at least a small shoulder bag (or 'butt pack') but when only carrying one body a flash and two lenses with film, I like these home made vests better than any other vest that I've ever bought for photography. Please note that surplus military uniform tops from the other nations work just as well, but aren't usually available in the same broad range of colours. Carpe Lumen, Michael E.
