As I understand it, when using flash fill, yes, you can use the spot meter 
for the ambient exposure, but it's average for flash. But if you want to 
use spot metering with the flash, it's only wise to do so with the F mode 
of the camera;, and not use the TTL flash function, or the sensor on the 
flash itself. The flash should be set to manual for such operation. This is 
better with off-camera flash for sure.
