I hold it with shutter up and the finder end cradled in my left hand. when I hold it shutter release down and cradled in my right palm, I must use my thumb for shutter release. My M6 has the TA Rapidwinder and this combination works best if the camera is held shutter release up, as the wind lever is awkward if you hold the camera shutter release down.
