This Leica RF user (since 1962) has had misaligned rangefinders on about
50% of new bodies. 2 out of about 15 have had near focus adjustment
problems (when the infinity adjustment was OK).

This has happened with M4's, M5's, M4-2's and M6's.

I've had 1 bad lens out of the box (out of about 30), due to sloppy
assembly which was fixable.

Fortunately, once fixed the pieces usually worked well with reasonable CLA.
