 We use Extensis Fetch (now "Portfolio") on a Mac. It has some slightly 
non-standard commands, but otherwise is stable and fairly fast. The 
catalog I use has more than 13,000 jpeg'd images in it dating from the 
end of 1995. It catalogs just about any image format, sounds, movies, and 
text.
 Fetch/Portfolio will automatically catalog a whole disk or folder of 
images. It does not extract caption information (Photoshop's File Info), 
but does extract keywords entered into Photoshop's File Info window. One 
can also add keywords to an image or group of images in the Fetch 
catalog. Good keywords are the key to keeping track of a large number of 
images. Spend some time coming up with a consistent and specific set of 
keywords.
