Can't tell from the picture, but I figured maybe some kind of helicoid added as part of the surgery? Maybe the whole lens rotates to focus, like an original 50mm Elmar? I can ask the next time I'm downtown in Leica Land, but it seems kind of academic. To me, the cost of adapting a 21mm or 28mm G-mount Biogon is so close to other, less complex alternatives (used Leica M-mount lenses, 28mm Ricoh GR L-mount, etc) that it doesn't seem worth the trouble.
