It happened to me twice. Well, not really twice. One of them was the  
dreaded Eskimo half roll. I was trying to sneak up on some illegal  
immigrant Canada geese with the Leica IIIc held up to my eye. I never  
noticed a huge wake from a powerboat. I lost the paddle and bailed  
out when the kayak reached the 180 degree point. Fortunately a had a  
wrist strap so I didn't lose the camera. The second time was in the  
Delaware River rapids when I misjudged a hole. The camera was on a  
neck strap. I finally came to the late realization that it is a dumb  
idea to use an expensive, non waterproof camera in a kayak.
