1)  As small as you can make it.

2)  Focal length depends on the lens mount to film distance.  Leica M or
Contax G would make shorter focal length pinhole cameras than would SLRs.
Quality would most likely depend on the precision of the pinhole.  See the
Calumet catalog.  They sell pre-made precision pinholes mounted into pody
caps for a number of cameras, giving the focal length and f-stop for each.
