Hi Arthur.  I have never been queried by Customs on camera equipment when
returning from trips to Europe (over 30 times in the last 10 years).  I do
on occasion take a photocopy of my insurance policy page of scheduled items,
which shows that certain valuables I have with me were insured prior to my
date of departure.  This has worked well so far for items such as watches
and jewelry, as long as the policy form shows specific data such as serial
numbers..
I investigated the procedure for the Customs form you mentioned, but went to
the insurance policy approach as it was less hassle.  At the time I
inquired, Customs required that I bring the item to them for their
inspection several days prior to departure.  This procedure may have changed
in the interim. Bob.
