This sounds like the synchronizing baseplate for the SELSY or SELIS 
flash.  The sliding switch adjusted the delay which varied according 
to the type of bulb used.  One of the imprinted numbers should be the 
serial number of the camera body to which the baseplate was mated.  
Both numbers given fall in the IIIc production range.
